- Schedule an appointment.
- Pre-visit check in will be sent via text 5 days before your confirmed appointment.
- Pre-visit check in will cover demographics, history, forms and policies.
- Bring the following to your scheduled visit:
- Current insurance card
- List of any medications your child is taking
- Immunization records
- Signed Medical Release Records form
